I have not been and will not be under the influence of drugs or alcohol within 24 hours of the appointment.

To my knowledge, I do not have any physical, mental, or medical impairment/disability that might affect my well-being as a direct or indirect result of my decision to have a tattoo.

I acknowledge that I may have allergies to procedural products such as but not restricted to "second skin", "Tegaderm" or other aftercare products, "green soap", inks, and pigments, and agree to accept any and all risks and reactions.

I understand that If my skin color is darker, the colors and contrast will not appear as bright as they do on light skin.

I understand that skin treatments and laser hair removal treatments prior to my tattoo appointment may alter and affect the healing and appearance of my tattoo.

I acknowledge that a tattoo is a permanent change to my appearance and agree that it is not my tattoo artist's responsibility to remove or change the tattoo if it did not yield the desired results.
 

I understand that tattoo healing can take 4 weeks or longer.

I acknowledge that infection is always possible, particularly in the event I do not take proper care of my tattoo or neglect to follow aftercare instructions that have been fully explained to me. I agree to follow my aftercare instructions properly although touch-ups may be required and will be done at my own expense.  I can request a paper or digital copy of the aftercare instructions at any time.

I am aware the artist has the right to change or cancel appointments at any time for emergencies, personal and health-related reasons, and that my tattoo appointment time and date may be subject to change. I will cooperate with booking within the artists' availability.

I have read and agree to comply with all policies, terms, conditions, and deposit regulations stated in emails and on the information sheet below.

I acknowledge that I have truthfully represented that the obtaining of the tattoo is my choice alone.

By signing below I consent that I have read the tattoo release form provided and agree to its terms.
